How do lingual braces work?
Lingual braces are bonded to the inner, tongue-facing surface of the teeth – the side no one sees. To anyone looking at you, the braces are completely invisible. It is the only fixed-appliance method offering one hundred percent discretion with no compromise in effectiveness.
In our practice we use the WIN Lingual system (DW-LingualSystems, Prof. Dirk Wiechmann, Germany) – one of the world’s most advanced fully personalised lingual appliances. Each bracket is made individually using CAD/CAM, precisely fitted to the unique shape of each tooth of the specific patient.
Advantages of WIN lingual braces

– No decalcification or staining on the labial surface

Disadvantages of lingual braces
– Higher cost than labial braces
– A few weeks of speech adaptation

A scientifically confirmed advantage – no decalcification

Since the brackets do not touch the labial surface, there is no plaque retention there and no risk of white spot lesions (WSL).
Wiechmann, Klang, Helms, Knösel (Am J Orthod, 2015)
They showed that the incidence of decalcification after treatment with the WIN appliance was markedly lower than results reported for labial braces.
Knösel et al. (Eur J Orthod, 2016) – 630 patients
It was confirmed that using the lingual enamel surfaces for bracket bonding not only has aesthetic advantages but may outweigh labial braces in terms of reducing decalcification.
Lazar et al. (J Clin Med, 2023) – narrative review
Of all types of fixed braces, the lowest risk of decalcification is associated with lingual appliances – WIN being the most effective, ahead of Incognito.
